Your application form can live at an address of your own instead of the standard link. There are two ways:
  • Embed it on your website. Paste one line of code into a page of the website you already have.
  • Give it its own page in your CRM. Build a one-page funnel in Sites, put the form on it, and connect your domain, so it opens at an address like application.yourdomain.com or apply.yourdomain.com.
Either way it’s the same form, with your logo and colours, and every application still becomes a deal. Setting it up needs access to Settings in NextLevel MCA, and to Sites and the CRM’s Settings for the second way.

Copy your embed code

Both ways start here.
1

Open the embed code

In NextLevel MCA, go to Settings → Application Form and open the Embed code tab.
2

Copy it

Under Embed code, click Copy embed code. It’s one line that starts with <script.
Settings → Application Form → Embed code: Copy embed code copies the line to paste.

Settings → Application Form → Embed code: Copy embed code copies the line to paste.

The form sets its own height as the merchant fills it in, so there’s no size to adjust. If Form active is off under Form behavior, the page shows Form Unavailable instead. See Application form and share link.

Option 1: Embed it on your website

Paste the embed code into the page where the form should appear. Most website builders have a block for this, called HTML, Custom HTML, Code or Embed. Publish the page, then open it to check the form shows. Use one form per page.

Option 2: Give it its own page in your CRM

Choose the address

  • A subdomain, such as application.yourdomain.com. This works wherever your main website is hosted, because only the subdomain points at your CRM.
  • A path on your main domain, such as yourdomain.com/application. This only works if yourdomain.com itself is connected to your CRM, for example because your website is built in Sites. If your website is somewhere else, use a subdomain.

Create the funnel

1

Open Sites → Funnels

In your CRM, go to Sites, open the Funnels tab and click New funnel.
2

Name it

Keep From blank, type a Funnel name such as Application, and click Create.
Sites → Funnels: New funnel is at the top right.

Add the page and set its path

1

The new funnel opens on its Steps tab. Click Add new step or import.
2

1

3

In Quick Add, search for code and click Code. A Custom HTML/Javascript box appears on the page.
4

Click the box, then Open Code Editor on the right. Paste your embed code and click Save.

Connect your domain

1

Open Domains

In your CRM, go to Settings → Domains & URL Redirects and click Connect a domain.
2

Choose funnels

Next to Funnel/Website/Store/Blog/Webinar, click Connect.
3

Enter the address

Type the full address you chose, such as application.yourdomain.com, and click Continue.
4

Add the DNS record

If your domain provider is supported, Continue sets the record up for you after you sign in to the provider. Otherwise click Add record manually. The CRM shows the record to add: for a subdomain, a CNAME whose host is the first part of the address (application) and whose value is sites.ludicrous.cloud. Copy them into the DNS settings wherever your domain is managed, then click Verify records.
5

Link it to the funnel

Once the record is found, choose your Application funnel and its Application page as the default page, and click Link Domain.
A new DNS record can take from a few minutes to a day to be found. If your domain’s DNS is on Cloudflare, set the record’s Proxy status to DNS only, or it won’t verify.

Publish and check it

1

Publish

Back in the page builder, click Publish at the top right and choose your domain. Publishing needs a connected domain, so connect it first.
2

Open the address

Open your new address. You should see your application form.

Who gets the deals

Applications from your own page go to the Default originator set under Settings → Application Form → Form behavior, like the direct link. To give each rep their own link, add their rep code to the page’s address: application.yourdomain.com/?rep=CODE. The Rep link generator on the Embed code tab gives you that part under Parameter only. Merchants who apply from that link are assigned to that rep, on your website and on a CRM page alike.

Common problems

Check the record was added where the domain’s DNS is actually managed, with the host exactly as shown (application, not the full address) and no other record for the same name. Give it time to spread, up to a day. On Cloudflare, set Proxy status to DNS only.
Connect your domain first, under Settings → Domains & URL Redirects, then publish again.
Open the Code element again and check it holds the whole embed code, from <script to </script>. If the page shows Form Unavailable, turn Form active on under Settings → Application Form → Form behavior.
The address the merchant opened didn’t end with ?rep= and that rep’s code. Check the rep has a rep code in Settings → Team, and that their link includes it.
